Membership features a low monthly contribution amount, and includes a very low annual Pre-Share amount ($500 for single, $1000 for a couple, and $1500 for a family), plus a Co-Share amount of 20% of the next eligible bills until your maximum amount of Co-Share is met (Single maximum is $1000, Couple maximum is $2000 and Family maximum is $4000). An additional pregnancy pre-share amount (Pregnancy Fee) of $900 for Couple memberships, and a $2500 for Single membership also applies).
Applicants with pre-existing medical conditions should make sure that they understand the limitations of the program for such conditions. During the first year of membership, there is no sharing for pre-existing conditions. For the second and third year of membership, sharing for pre-existing conditions is limited to $25,000. During the fourth and subsequent years of membership, such conditions are no longer considered pre-existing. Members with certain conditions or lifestyles may be required to participate in the WellCheck Assist program or WellCheck Support program for an additional monthly charge. If you are assigned to one of these programs, and do not want to continue your membership, you are entitled to a full refund of your application fee.
Please note that for medical incidents that occur during the first 60 days of membership, total sharing is limited to a maximum of $25,000, and those medical needs must be a result of an acute illness, injury, or accident.

If you are accepted as a member, your membership effective date will be the first day of the month following your acceptance.
